*** Paperwork should only be completed by individuals who have called and scheduled an appointment with a provider. Our office will provide a password so you can open the protected pdfs below.
Prior to your first appointment, please click on the appropriate link below to download the New Patient Intake paperwork. Save the file to your desktop, and fill in the fields using Adobe Acrobat.
Send the files to Intake@wandrcenter.org
If you have already completed the Intake paperwork but need to complete a Release of Information, click the link below. This is ONLY a Release of Information. The Registration paperwork is still required.
